The following five Items are required when applying for a U.S. passport

1.    Properly completed application
2.    Proof of child's relationship to parents/guardians (under age 14)
3.    Proof of U.S. citizenship
4.    Proof of Identity
5.    Payment of fees

You must use form DS-11 if:
1.        You have never had a U.S. passport or
2.        You are under age 16 or
3.        Your most recent passport was issued more than 15 years ago or
4.        Your most recent passport was lost or stolen.

If you are applying for a passport, using form DS-11, because your most recent, valid passport was lost or stolen, you must also submit form DS-64.  

You can apply for a passport by mail using Form DS-82 if:
1.        You can submit your most recent passport AND
2.        You were at least 16 years old when your most recent passport was issued AND
3.        You were issued your most recent passport in the last 15 years AND
4.        You use the same name as on your most recent passport or since your passport was issued, you have had your named changed by marriage or court order and can submit proper documentation to reflect your name change.

TWO IDENTICAL PASSPORT PHOTOGRAPHS:  2x2 inches in size, identical, taken within the past 6 months showing current appearance, color, full face, front view with a plain white or off-white background, and taken in normal street attire.  No uniforms, hats or headgear or dark glasses unless required for medical reasons (a medical letter may be required).

Cost:  $7.99 + tax for 12 photos. Photos are developed immediately.  Be sure to tell the photo assistant, BEFORE having your picture taken, that you are affiliated with Davidson College otherwise you will only receive 6 photos for $7.99 + tax. 

EXPEDITED SERVICE - For faster processing, you may request Expedited Service.  Expedited requests will be processed in about three weeks from the date you apply.  The additional fee for expedited service is $60 per application.  Expedited Service is available only in the United States.
